Intake Procedure

To schedule an appointment, please call (803) 567-0064. If we are unavailable, please leave a message and your call will be returned within 24 business hours.
Upon scheduling your appointment, you will receive an email with a link directing you to the intake paperwork to complete online. The intake paperwork must be completed the day before your appointment or your appointment may be canceled. An automated text reminder will be sent by our system reminding you of your appointment 24 hours that is scheduled for tomorrow. Your first session is the initial evaluation and will approximately last 50-60 minutes. Please arrive 10-15 minutes before your scheduled time to allow for any additional paperwork to be completed. If you are being seen virtually, the link will also be included in the text reminder.

Contact InfoNon-emergency calls should be limited to regular business hours and will be returned during normal business hours as outline on the website. Most calls are returned within 24 hours. If you are experiencing a life-threatening emergency, in danger of hurting yourself or someone else, feeling suicidal, overwhelmed, or in crisis outside of normal business hours, please do one of the following:
– Contact 911
– Go to your nearest hospital emergency room
– Call the National Suicide Prevention Lifeline at 1-800-273-8255
Fee Policy

Evans Counseling and Consulting is committed to providing you the best possible care. Payment is due at time of service and accepted methods of payment include cash, debit card, and credit card. A credit card is required to be on file.
Our automated system processes payments for all appointments scheduled that day. If payment is not obtained or declined, the appointment will be canceled.
Fees for administrative services will be billed, if requested, at half of the individual therapeutic rate for a minimum of 60 minutes. This includes, but is not limited to treatment summary requests, professional letters, work forms, and phone/conference calls.
Evans Counseling & Consulting does accept insurance, however, not every service can be paid by insurance. Couples/Marriage Counseling and Immigration Evaluations are self-pay only. We accept the following insurances:
- UnitedHealthcare (UMR, GEHA)
- BlueCross BlueShield (Regular, State, Federal, PAI)
- Medicare
- Aetna
- Cigna
Every effort will be made to verify your insurance benefits and provide an approximate cost for services as determined by your health insurance plan.
Cancellation Policy
Learn more about our cancellation protocol and procedures to avoid charges due to missed and late canceled appointments.
If you are unable to keep your appointment, please provide a 24-hour notice to avoid the cancellation and no-show fee. Failure to provide a 24-hour notice prior to your scheduled appointment time will result in a $50 late cancellation fee. A $100 no-show fee will be charged if you fail to show for your scheduled appointment. Clients are given an annual “free pass” that can be used to waive one late cancellation fee. If you late cancel but reschedule within the same week, no fee will apply.
Clients who have not scheduled an appointment in 4 weeks will be considered terminated and availability for scheduling will be determined by the counselor’s current caseload.
Clients who cancel or frequently reschedule their appointments may be terminated at-will. It will be assumed that you are not ready for therapy or to continue in the therapeutic process.
